Case Description
About a year ago, I started a habit of teeth grinding (bruxism) during sleep, and for a while earlier I was biting my lips quite often when feeling stress. By nature, I like eating hard-to-chew food. On waking up, I used to feel that the two muscles near the eyes are stressed for some time. Then, I started to hear a sound of cracking in the right jaw joint with a reduced ability to chew food as if jaw muscles were not strong enough. Then, there appeared a small distance between the upper and lower teeth with a slight pain and inability to bite thin things. It was diagnosed as right jaw arthritis. Thus, I started to eat easy-to-chew food.

Now, the condition of the left side of my jaw is getting worse, my ability to chew food is deteriorating, the distance between the upper and lower teeth is getting wider and the difficulty in speaking is getting worse.

I have made a Magnetic Resonance Imaging (MRI) of both Temporomandibular joints and a panoramic scan which have shown an intervertebral disc of Temporomandibular joints. Then, I started to take medications, but in vain.

Now, there is such a severe muscle strain in the area beside the eyes that the eyebrows bones started to be protruding. Then, a device has been fastened to my mouth to keep it open for two months. During sleep, I started to feel as if there is paresthesia in my lower jaw, for some time. Then, a molar on the left side of the jaw which was the worst due to the muscle strain, started to tilt.

Then, there started to be sever pains in the neck, and a sound was heard with the movement of the neck.

The condition is getting worse and the distance between the upper and lower jaw is getting wider.

Dentists began to find out why the lower jaw lowered, making scans to detect if the lower jaw grows in size or there are other reasons - theses scans shall be attached to the file. But, these scans found no abnormality. A model of occlusion has been made,

After a while, I felt a severe pressure in the lower part of the jaw as if jaw muscles are getting so weak that the jaw is shaking, there are muscles moving in the face, distance between the upper and the lower jaw is getting wider, mouth opening is limited and the jaw hardly moves when talking. Besides, my ability to chew food, which was already deteriorating over the last year, has vanished completely. It is also noticed that the jaw dose not move to the right or to the left. A second MRI was performed, and will be sent as well.

What is the diagnoses? Is another scans checks required for the diagnosis? If you have a cure, what are the costs of such a cure?

When selecting a new doctor, consider the following:
  • Quality of Explanations: Compare dentist replies. Give preference to well-reasoned, detail-oriented, transparent explanations.
  • Experience & Qualifications: If a dentist has a practice listing, check their practice description, special training, typical office procedures and before & after case photographs.
  • Other Patients' Freedback: If a dentist has reviews, read other patients' reviews. The more reviews they have, the more likely it is that other patients' feeback is accurate.
  • Treatment Costs: Dental procedure costs vary from practice to practice and depend on a variety of factors. Because quality of patient care and treatment results vary from practice to practice most dentists will not "price-match" treatment fees of others.

Wow... you have had a really difficult journey. TMJ problems really are the most painful dental problem you can have. It sounds like you have had a lot checked and investigated so far. To answer your question, your situation is impossible to diagnose without seeing those scans and examining you (it is also impossible to estimate the costs right now as well).

Here''''s what I would consider, IF you have a great team treating you well, keep asking questions and make sure they know how you are progressing (or not). If you are NOT making progress, then it might be time to consider a different approach.

My approach would be to get you out of pain as quickly as possible. Once you are feeling better, then we have to determine where your jaw joint should sit, as well as how your teeth should align.

For now: keep following the recommendations your team has given you, take your prescriptions, manage your diet (probably liquid), manage your stress, and sleep right. Unfortunately, things can still get worse.

Stick with it, there is a solution out there!

Obviously you have a severe problem. Unfortunately this problem is impossible to diagnosis or treat through email. Unfortunately, there are too many unanswered questions. My best advice would be to find a dentist who is comfortable and has the qualifications to treat you or refer you to someone who can. Our practice utilizes a conservative noninvasive approach for initial pain management.

Hope this helps
